    Assessment of applicability and transferability of evidence-based antenatal interventions to the Australian indigenous setting

    There is a need for public health interventions to be based on the best available evidence. Unfortunately, well-conducted studies from settings similar to that in which an intervention is to be implemented are often not available. Therefore, health practitioners are forced to make judgements about proven effective interventions in one setting and their suitability to make a difference in their own setting. The framework of Wang et al. has been proposed to help with this process. This paper provides a case study on the application of the framework to a decision-making process regarding antenatal care in Aboriginal and Torres Strait Islander communities in Queensland. This method involved undertaking a systematic search of the current available evidence, then conducting a second literature search to determine factors that may affect the applicability and transferability of these interventions into these communities. Finally, in consideration of these factors, clinical judgement decisions on the applicability and transferability of these interventions were made. This method identified several interventions or strategies for which there was evidence of improving antenatal care or outcomes. By using the framework, we concluded that several of these effective interventions would be feasible in Aboriginal and Torres Strait Islander communities within Queensland

    Interventions to improve cultural competency in healthcare: A systematic review of reviews

    BACKGROUND: Cultural competency is a recognized and popular approach to improving the provision of health care to racial/ethnic minority groups in the community with the aim of reducing racial/ethnic health disparities. The aim of this systematic review of reviews is to gather and synthesize existing reviews of studies in the field to form a comprehensive understanding of the current evidence base that can guide future interventions and research in the area. METHODS: A systematic review of review articles published between January 2000 and June 2012 was conducted. Electronic databases (including Medline, Cinahl and PsycINFO), reference lists of articles, and key websites were searched. Reviews of cultural competency in health settings only were included. Each review was critically appraised by two authors using a study appraisal tool and were given a quality assessment rating of weak, moderate or strong. RESULTS: Nineteen published reviews were identified. Reviews consisted of between 5 and 38 studies, included a variety of health care settings/contexts and a range of study types. There were three main categories of study outcomes: patient-related outcomes, provider-related outcomes, and health service access and utilization outcomes. The majority of reviews found moderate evidence of improvement in provider outcomes and health care access and utilization outcomes but weaker evidence for improvements in patient/client outcomes. CONCLUSION: This review of reviews indicates that there is some evidence that interventions to improve cultural competency can improve patient/client health outcomes. However, a lack of methodological rigor is common amongst the studies included in reviews and many of the studies rely on self-report, which is subject to a range of biases, while objective evidence of intervention effectiveness was rare. Future research should measure both healthcare provider and patient/client health outcomes, consider organizational factors, and utilize more rigorous study designs

    ā€œIt Depends How Youā€™re Saying Itā€: The Complexities of Everyday Racism

    While racism is widely recognised as a complex social phenomenon, the basis for defining and identifying everyday racism from a lay perspective is not well understood. This exploration of factors used to frame everyday racism draws on seven cognitive interviews and four focus groups conducted in November 2010 and January 2011 with Australian adults predominantly from Anglo ethnic and cultural backgrounds. The study reveals lay theorising centring on tropes of intentionality, effect of speech, relationality and acceptability. Participants were more likely to think of racism as having negative, overtly offensive and emotional connotations. Racialised speech that was not considered to be blatantly racist was more contested, with participants engaging in complex theorising to determine whether or not such speech constituted racism. The study also highlights the potential of qualitative research to inform survey development as an unobtrusive method for in-depth participant reflection. The ambiguous nature of everyday racism demonstrated in this paper indicates a need to foster more nuanced lay understandings of racism that encompass the subtle, rational and complementary expressions that can be situated within institutions and society

    Health behaviours are important determinants of health and adoption of unhealthy behaviour is considered as one of the mechanisms through which immigrants\u27 health changes over time in the host country. The change in health behaviours over time can contribute either to improving or worsening the overall health status of immigrants. Despite being the important mediators for the change in overall health status and chronic health conditions, no previous review (either general or systematic) has examined differences in key health behaviours simultaneously between immigrants and non-immigrants. This study aims to provide a systematic overview of the current global literature on differences in key health behaviours (that is, tobacco smoking, physical activity and alcohol drinking) between immigrant and non-immigrant groups

    Exploring relationships between racism, housing and child illness in remote indigenous communities

    Background Although racism is increasingly acknowledged as a determinant of health, few studies have examined the relationship between racism, housing and child health outcomes. Methods Cross-sectional data from the Housing Improvement and Child Health study collected in ten remote indigenous communities in the Northern Territory, Australia were analysed using hierarchical logistic regression. Carer and householder self-reported racism was measured using a single item and child illness was measured using a carer report of common childhood illnesses. A range of confounders, moderators and mediators were considered, including socio-demographic and household composition, psychosocial measures for carers and householders, community environment, and health-related behaviour and hygienic state of environment. Results Carer self-reported racism was significantly associated with child illness in this sample after adjusting for confounders (OR 1.65; 95% CI 1.09 to 2.48). Carer negative affect balance was identified as a significant mediator of this relationship. Householder self-reported racism was marginally significantly associated with child illness in this sample after adjusting for confounders (OR 1.43; 95% CI 0.94 to 2.18, pĀ¼0.09). Householder selfreported drug use was identified as a significant mediator of this relationship. Conclusions Consistent with evidence from adult populations and children from other ethnic minorities, this study found that vicarious racism is associated with poor health outcomes among an indigenous child population

    The importance of reconciliation in education

    The aim of this paper is to summarise existing evidence and new analyses that shed light on the role of reconciliation in schools and early learning services in particular, and in education more broadly. We present the first analysis in Australia of the relationship between racism/discrimination and cognitive development among the Indigenous Australian population, showing a negative and statistically significant longitudinal relationship. We discuss the policy implications of these findings, as well as the implication of the broader literature on reconciliation in schools and early learning services
